      <description>&lt;table cellpadding="0" cellspacing="0" border="0" id="tabcolumn-1" style="width: 100%; margin-bottom: 15px"&gt;&lt;tr&gt;&lt;td&gt;&lt;div id="column-1" usermodifiable="true" style="width: 100%"&gt;&lt;div id="ctrl-9613338"&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Enrollment:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;I have important paperwork that I will need you to complete before I can begin to have &lt;br&gt;your child in my care. &amp;#160;I will need you to keep me informed of any address or phone &lt;br&gt;number changes in case I need to reach you. &amp;#160;No enrollment fees or deposits required to &lt;br&gt;begin care!&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Open Door Policy:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;You are welcome to come to my home anytime during my regular daycare hours. &amp;#160;I will &lt;br&gt;keep my doors locked for the safety of the children, but just knock and I will quickly let &lt;br&gt;you in.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Late Fee:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;If you will be late picking up your child, you need to call me and let me know. &amp;#160;There will &lt;br&gt;be a late fee of $10.00 per 15 minutes added onto your usual weekly fee if you are late &lt;br&gt;without letting me know in advance.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Payment:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;Payment for childcare is due each Monday morning prior to the week's care. &amp;#160;A $10 fee is &lt;br&gt;added onto the regular week's payment for each day that payment is late. If I am given a &lt;br&gt;check that bounces, then there will be a $30 fee added onto your usual payment (due to &lt;br&gt;charges that the bounced check causes with the bank). If a second check bounces then I &lt;br&gt;will only accept cash payments from then on. &amp;#160;If payment continues to be a problem, then &lt;br&gt;care could be terminated.&lt;br&gt;&lt;br&gt;If the daycare is closed due to a holiday, your weekly payment is still due in full for that &lt;br&gt;week. &amp;#160;If the daycare is closed for 1-2 days in one week, your weekly payment is still due &lt;br&gt;in full for that week (*If the child is in the daycare at least 4 days in a week, the full flat &lt;br&gt;rate is always due in full*). &amp;#160;If the daycare is closed 3 days or more in a week, then your &lt;br&gt;weekly rate will be prorated. &amp;#160;If the daycare is closed all week for a personal vacation, no &lt;br&gt;payment will be due. &amp;#160;I will always notify you in advance of any daycare closures.&lt;br&gt;&lt;br&gt;If your child is out sick, your payment is due in full for that week as well. &amp;#160;&amp;#160;If you are &lt;br&gt;planning a vacation please let me know at least 2 weeks in advance. &amp;#160;You are allowed one &lt;br&gt;week per year of not paying for 1 week if your child is absent. &amp;#160;Any additional weeks of &lt;br&gt;absence, then you must pay your regular fees in order to hold your child's space. (Your &lt;br&gt;&amp;quot;year period&amp;quot; begins on your anniversary date of when your child begins coming to the &lt;br&gt;daycare). &amp;#160;These rules are for full and part time children. &amp;#160;By paying your regular daycare &lt;br&gt;fee each week, you are holding your child's space. &amp;#160;&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Immunizations:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;Your child's immunizations MUST be completely up to date before coming to the daycare.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Hygiene:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;I ask that the children arrive bathed and smelling fresh and clean with a clean diaper and &lt;br&gt;clothed appropriately each day in daytime clothing. Please keep the child's fingernails &lt;br&gt;trimmed to avoid scratching of themselves, the other children and the furniture.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Meals:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;I provide breakfast, snack and lunches and drinks for the children. &amp;#160;This is included in your &lt;br&gt;weekly fee. &amp;#160;The only things I do not supply as far as meals is: &amp;#160;I do not provide formula &lt;br&gt;for the infants or jars of baby food. If you have an infant that is taking formula or specific &lt;br&gt;types baby food, then you would need to maintain a supply for him/her.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Sickness Policy:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;Please do not bring your child to the daycare if he or she is sick. &amp;#160;Please call me by 8am if &lt;br&gt;you're child is not coming that day. &amp;#160;A doctor's note will be needed to show me that your &lt;br&gt;child has been cleared to be around the other children if a contagious type of sickness has &lt;br&gt;occurred. I will call you to pick up your child if I believe that they are too ill to be in the &lt;br&gt;daycare. &amp;#160;Your child can return to the daycare when he or she is no longer contagious &lt;br&gt;(usually 24 hours after the symptoms are completely gone and/or 24-48 hours after &lt;br&gt;antibiotics have begun depending on the illness). &amp;#160;&lt;br&gt;&lt;br&gt;Your child cannot come to daycare with a fever of 100&amp;#186; or higher and the child must be &lt;br&gt;fever free for a minimum of 24 hours before returning to daycare. &amp;#160;That means fever free &lt;br&gt;without the aid of a fever reducing substance. Giving your child Tylenol to reduce the &lt;br&gt;fever before arriving to daycare in the morning is unacceptable because the fever will &lt;br&gt;reappear once again and the child will still have to be sent home. &amp;#160;If the child has a fever &lt;br&gt;of 100&amp;#186; or higher, I will call you to pick up your child as soon as possible.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;i&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Reasons why the child cannot come to daycare:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/i&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#ff3333"&gt;*&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#3333ff"&gt;&amp;#160;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;Fever of 100&amp;#186; of contagious illnesses of when your child needs to stay out of &lt;br&gt;daycare. &amp;#160;Each individual case is different and may require more or less time out ~ but will &lt;br&gt;always require a doctor's note to return stating that the child is totally well and &lt;br&gt;non-contagious in order to return. &amp;#160;If you feel that your child is sick with a contagious &lt;br&gt;illness, please verify with a doctor that your child is clear to come to daycare before &lt;br&gt;bringing him/her to the daycare. &amp;#160;This is for the protection of your child, all other daycare &lt;br&gt;children present and the daycare provider. &amp;#160;&lt;br&gt;&lt;br&gt;A doctor's note will show that you're child is cleared by a doctor, but you &lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#0033ff"&gt;must&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt; abide by &lt;br&gt;the daycare sick policies first, regarding when a child can or cannot come to daycare. It is &lt;br&gt;my hope to keep the daycare as germ-free and sick-free as possible. &amp;#160;Once ONE child &lt;br&gt;comes to the daycare sick, he or she is exposing everyone to that illness. &amp;#160;It can be a &lt;br&gt;horrible sick cycle! So lets keep it healthy. &amp;#160;If a child continually comes to the daycare sick &lt;br&gt;and the parents do not abide by the daycare sick policies, then childcare will be &lt;br&gt;terminated by the daycare provider.&lt;br&gt;&lt;br&gt;&lt;/font&gt;&lt;/b&gt;&lt;b&gt;&lt;u&gt;&lt;font size="4" color="#ff3333"&gt;Potty Training:&lt;br&gt;&lt;/font&gt;&lt;/u&gt;&lt;/b&gt;&lt;b&gt;&lt;font size="4" color="#0033ff"&gt;I will make every effort to potty train your child if you would like me to do so. &amp;#160;I will begin &lt;br&gt;when the child appears to be ready for this step. &amp;#160;Usually it is around 2 or 3 years old. &amp;#160;I &lt;br&gt;will ask that your child wear pull-ups during this period of potty training and I will only &lt;br&gt;attempt to potty train if the child is being potty trained at home as well. &lt;/font&gt;&lt;/b&gt;&lt;/div&gt;&lt;/div&gt;
